Support Coordination is a funded service that assists individuals in gaining access to needed programs and state plan services. In the Medicaid-based, fee-for-service system, all eligible individuals who wish to access DDD services must work with a Support Coordination Agency qualified by the DDD.

Your NJSS Support Coordinator will have a sustainable, manageable caseload that allows them to spend quality time and devote careful attention to your needs. Support Coordinators work closely with individuals and their families to develop their Person-Centered Planning Tool (PCPT) and Individualized Service Plan (ISP), updated at least once per year.

In addition to helping identify your support needs and preferences, your Support Coordinator will assist you in connecting with local, state, and federal services. These services include traditional disability providers (residential housing, day habilitation programs, etc.), generic community supports, government support beyond DDD, and/or natural support based on funds available in your budget.

Meeting your needs is the main objective.

Your Support Coordinator will provide ongoing monitoring of your supports and services and, at minimum, monthly check-ins.

They will always be readily available to respond to emergencies and other service-related needs.
